Added follow-up email control on payment schedules

Payment Schedules now accepts an optional FOLLOWUP field on Create Schedule SUBTYPE=CS). It overrides which follow-up emails the gateway sends to the cardholder for that schedule, including switching them off entirely.

FOLLOWUP can only be set when the schedule is created and cannot be changed afterwards.
